IN = In Network
OON = Out Of Network
*BCBS — Blue Cross Blue Shield (IN for most contracts, often means that you will not have to meet your deductible when seeking services here) (Blue Choice Behavioral Health Network, EPS, Expanded Psychiatric, PEEHIP Behavioral Health, State and Local Government Behavioral Health, FEP, PPO)
*Tricare (Registered Provided but OON for Prime)
*United Healthcare (OON for most contracts)
*Other insurances are accepted. (My services may be considered OON for your insurance company.)
Some insurance companies will pay toward your services with me despite my being OON and some insurance companies will not pay any OON benefits. I will be happy to be your mental health provider if you choose to work with me despite my being OON for your insurance company. However, please know that it is sometimes to your advantage to use an IN provider because it is possible that your deductible and per session copay will be lower.
Cash, check, medical spending accounts, debit and credit cards are accepted forms of payment.
We will be happy to file your insurance company claims through this office. We cannot guarantee if or what your insurance will pay. You are strongly urged to contact your insurance company to ask what is your benefit coverage. The phone number to call is most often on the back of your insurance card (sometimes listed as Behavioral Health/Services). Here is a page you can printout to guide you in asking your insurance company the questions that will give you some information to help you in making decisions related to financial considerations in seeking mental health services.

Contacting Dr. Sack
Please call 334-590-8492, during normal business hours, for making, changing and cancelling appointments, and for contacting Dr. Sack. There is no office assistant.
Outside of office hours, or during office hours when I am with someone, please leave a message for a return call. If you leave a non-emergency message and I do not return your call within 3 hours, please do not hesitate to call again. If you leave an emergency message I will make every effort to contact you immediately. However, if I don’t get back to you in the time frame of your needs then, please, be kind to yourself and go to the emergency room, call the National Suicide Prevention Lifeline at 1-800-273-8255, call the Crisis Hotline at 1-800-724-7240, call 911, or call someone in your life for support.
Please note that, to protect confidentiality from others who may have access to your phone or message system, calls to you from me may sometimes show up on your caller id as being from an Unidentified Caller.
An hours block of time is reserved for a complete focus on one person. No one else can be scheduled in that same time block. As a result, I kindly ask that, if you are going to be unable to use time saved for you, please call and give as much advance notice as possible that you will be canceling your appointment. Your thoughtfulness then opens that time block for someone else who might want that particular block.
No Shows and cancellations with less than a 24 hour advance notice will be billed to you as a regular session. Please be aware that insurance companies will not pay for these unused appointments
