Forms can be mailed to Dr. Sack at 1001 Fredericks Rd #21128, Catonsville MD 21228. Forms can also be sent by your encrypted email provider or by encrypted email when you give Dr. Sack your email address and you respond to Dr. Sack’s email to you with the forms as attachments to your response.
