Fees For Services

Fees for psychological care include $225 for the first appointment (CPT 90791), and $175 for subsequent appointments (individual - CPT 90834 and family - CPT 90847). Like many psychologist, Dr. Sack does not participate as an in-network (IN) provider with insurance companies. (Registered & Authorized Tricare Provider: Non-Network)

Out Of Network Insurance Coverage

Many insurance companies will pay you back for psychological services you pay for, even if the care provider is out-of-network (OON). Some insurance companies will not pay any OON benefits. You may choose to take care of your financial interests by calling your insurance company to identify what benefit coverage you have. This includes information about whether they will reimburse for OON providers, whether they pay for telehealth with OON providers, whether preauthorizations are needed, what is your deductible, what percentage of the per session fees the insurance will pay, and what is their usual and customary fee limits per session. Here is a page you can printout to guide you in asking your insurance company question about your insurance coverage.

Dr. Sack will be happy to be provide care to you if you choose to seek her care despite being OON for your insurance company. However, please know that it is sometimes to your financial advantage to use an IN provider because it is possible that your deductible and per session fee will be lower.


FILING OF CLAIMS

Whenever possible, Dr. Sack’s office will make efforts to file your insurance company claims for you. We cannot guarantee if or what your insurance will pay. Any follow-up calls to insurance companies will be the responsibility of the patient. If you would prefer to, or if Dr. Sack’s office is unable to, file your own insurance claims the information you need to file these claims will be provided to you.

PAYMENTS

As a courtesy to patients, Dr. Sack accepts credit/debit cards, Health Service Accounts (HSA) cards and Flex Spending Cards for payment at the time of service. Although, for in-person sessions, check or cash payments are encouraged at the time of service.

Outside of office hours, or during office hours when Dr. Sack is with someone, please leave a message for a return call. If you leave a non-emergency message and your call is not return within 3 hours, please do not hesitate to call again. If you leave an emergency message every effort will be made to contact you immediately. However, if you are not called in the time frame of your needs then, please, be kind to yourself and go to the emergency room, call the Suicide and Crisis Line at 988, call 911, or call someone in your life for support..

To assist Dr. Sack in knowing something about you prior to your first session, please download, read, fill out, and send these forms to Dr. Sack prior to your first visit. Forms can be mailed to Dr. Sack at 1001 Fredericks Rd #21128, Catonsville MD 21228. Forms can also be sent by your encrypted email provider or by encrypted email when you give Dr. Sack your email address and you respond to Dr. Sack’s email to you with the forms as attachments to your response.
